Forms and Fees 2017-18
Please NOTE - All students must apply and have been accepted into a Protégé course before the PO can be placed. After parents or guardians have completed the Online Application we will send you a confirmation email and the PO can then be submitted.
We will be taking applications for second semester soon!

There will be a $20 late fee for late registration fees and/or late tuition payments.
We appreciate you paying on time. It helps us be ready in every way when classes start.
Registration Forms & Process:
1. Please complete the Online Application.
2. After you do so, you will receive an email notification of whether your application has been approved. Upon approval, please continue with the process.
3. Mail the $75 registration fee with these three forms:
Medical Release Form
Liability Release Form
Behavior Contract
3. You will receive a confirmation email and tuition invoice once we have received your forms and registration payment.
4. Any PO’s need to be received by September 29, 2017 and January 31, 2018.
